For client sites and inpatients that send bottles to be incubated at our facility that are not yet known positive.

Methodology:

Organism Identification is performed by biochemical test and/or Bruker MALDI-TOF mass spectrometry RUO Database. Organism sensitivities (AST) are performed as applicable using Broth Microdilution and/or Gradient diffusion strips using FDA approved methodology and CLSI guidelines. Performance has been evaluated and verified by UnityPoint Health - Des Moines, Pathology Laboratory.

Interpretive Data:

A Biofire Filmarray Blood Culture Identification multiplex PCR test will be performed, if gram stain morphology indicates, on the first bottle to turn positive on a patient in a 72 hour period. Testing will be performed on only one bottle from a set unless the Gram stain on the second bottle shows a different morphology.

Collection Information:

Aseptic technique must be used in obtaining, and transport of specimen.
Patient preparation:
1. Clean venipuncture site with Chloraprep swab for 15 seconds. Allow to dry for 30 seconds. The site must be dry before performing the venipuncture. Do not touch the site with any non-sterile object. Do not re-palpate the site.
2. Disinfect the top of the bottle stopper with 70% isopropyl alcohol wipe prior to collection.

Minimum Sample Volume: Minimum amount of analytical sample (serum, plasma, urine, CSF, etc.) required to perform an assay once. Submitting the minimum volume makes it impossible to repeat the test or perform confirmatory or reflex testing. In some situations, a minimum specimen volume may result in a QNS (Quantity Not Sufficient) result, requiring a second specimen to be collected.

Clotted, improper container or Does not meet collection, storage or transport requirements.
No more than 3 blood cultures will be accepted per patient per 24-hour period unless authorized by Microbiology Manager, Pathologist, or Director of Laboratory Services
